What You'll Do
- →The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) is responsible for providing competent primary care to patients and oversight of licensed practical nurses.
- →This nurse provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department.
- →Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels.
- →This nurse provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management.
- →The nurse will assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization.
- →Demonstrates leadership in delivering and improving holistic, primary care through collaborative strategies with others.
- →Evaluates practice in an ongoing process, based on best evidence.
- →Partners with others to effect change and produce optimal outcomes.
- →Supports colleagues through knowledge sharing to provide safe, quality outpatient nursing care.
- →Duties include but are not limited to: Using the nursing process and evidence-based practice to collaborate with the Team (family/caregiver, internal and community-based services involved in providing care to the patient) in developing and patient -driven holistic care plan for life.
- →Providing patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's goals.
- →Following the patient's personalized health care plan as designated by the patient and Patient Aligned Care Team (PACT).
- →Demonstrating advanced clinical knowledge in assessing planning, implementing, documenting, and evaluating care for a designated group of patients across the continuum of care.
- →Triaging and applying a collaborative team approach in identifying, analyzing, and resolving patient care problems.
Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
- ✓Applicants who refuse to be tested will be denied employment with VA.
- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓Subject to background/security investigation.
- ✓Selected applicants will be required to complete an online onboarding process.
- ✓Acceptable form(s) of identification will be required to complete pre-employment requirements (https://www.uscis.gov/i-9-central/form-i-9-acceptable-documents).
- ✓Effective May 7, 2025, driver's licenses or state-issued identification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment.
- ✓Must pass pre-employment physical examination.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
- ✓You may be required to serve a probationary period.
