About the Role

The Las Vegas Residential, Renewal, and Rehabilitation (LVR3) Admissions and Discharge Coordinator independently provides screening assessments, clinical psychosocial assessments, treatment planning, measurement-based care, discharge planning, evidenced-based on individual/couples/family counseling, group counseling, care coordination and case management services.

What You'll Do

  • Total Rewards of a Allied Health Professional The admissions and discharge coordinator provides the highest level of customer service and care coordination to the veteran, the referring provider and others involved in the veteran's care.
  • Veterans receiving care from the LVR3 Domiciliary program traditionally have complex health care and psychosocial problems and needs, requiring a high degree of clinical oversight and creative problem solving.
  • Services are provided in a residential mental healthcare setting.
  • Additionally, the Senior Social Worker, Admissions and Discharge Coordinator can serve as a mentor to other social workers, particularly those at the entry level.
  • Duties may include but are not limited to: Arranges referrals or services on behalf of the veteran to assure that necessary and timely clinical services are received, both for VA and community care residential treatment services.
  • Interviews veterans and their family members/significant others, as well as appropriate clinical personnel, to establish facts about the veteran's situation.
  • Coordinates day-to-day functions of the admissions and discharge program that treats a variety of clinically challenging patients.
  • Assist Domiciliary staff with completing psychosocial assessments, treatment planning, case management, discharge planning, measurement-based care, suicide assessment and safety planning, individual counseling, and discharge planning.
  • Provide individual, group, and family counseling and advanced level case management interventions used in the treatment of veterans with mental health diagnoses, substance use disorders, traumatic brain injuries, poly-trauma injuries, visual impairment, post-traumatic stress disorder, and other serious disorders.
  • Responsible for carrying out/implementing all social work functions/duties in the Domiciliary, including promotion of social work services as a key component of treatment.
  • Provides social work services to veterans and family members/significant others in support of the veteran's treatment.
  • Establishes a continuing relationship with the veteran, evaluating progress towards goals and adjusting the treatment plan as appropriate.
  • Offers consultation to colleagues and students on the psychosocial treatment of patients treated in specialty areas, rendering professional opinions based on experience and expertise and role modeling effective social work practice skills.
  • Participates fully in peer review, assessing the quality of professional social work practice standards provided in the organization.
