About the Role

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

What You'll Do

  • The supervisor provides direct clinical services in the Renal Dialysis Clinic and will be responsible for a reduced patient care caseload commensurate with the scope of supervisory responsibilities as assigned.
  • The Social Work Supervisor is licensed or certified at the advanced practice level and must be able to provide supervision for subordinate staff to qualify for licensure.
  • The Renal Coordinator is responsible for assisting Veterans receiving renal/nephrology care at Columbia VA Health Care System.
  • The coordinator serves as dialysis/renal point of contact for the Greenville OPC as well the CBOC's.
  • The coordinator provides clinical psychosocial and case management services.
  • The coordinator serves as a point of contact for all stakeholders including Veterans, families, community hospitals, nursing homes and community dialysis clinics.
  • The coordinator is responsible for program management for Fee Basis/NVCC dialysis.
  • A high degree of knowledge and clinic skills is necessary for this position due to the complex medical needs of Veterans served.
  • The incumbent must have specialized knowledge of dialysis issues, transplant issues, working with people with chronic illnesses and end of life issues.
  • As the supervisor, the incumbent assists in planning, establishing, and implementing policies and procedures; developing individual or group goals and objectives as well as monitoring, operating, evaluating, coordinating, and overseeing the Social Work staff of the assigned program areas.
  • The supervisor is responsible for monitoring staff on in- patient Mental Health to ensure timely psychosocial assessments and discharge planning.
  • This includes monitoring the Utilization Management Report daily and tracking all patients pending discharge for social issues.
  • The SW supervisor will meet routinely with staff and assist with recommendations to ensure ongoing care coordination.
  • The incumbent will assure the Social Workers whom they supervise are aware of, adhere to all current directives of VHA, and meet current Joint Commission standards, State licensure standards, and VA-OIG expectations.
